VIP! "Member" or "Client" Qualification!

Section I. Overview! *


aHAND4 Life! ® is firstly a "Membership Not-for-Profit Organization", meaning its Operational Policies, and thereby, its future (i.e. "governance") is determine by its Voting Membership, by its Directors. There are multiple Classes established to create the most effective Organization possible to Serve/Support VIP!s in perpetutity!

One significant Class of Membership who we wish to have intimately participate in the Governance of our Organization, are from a specific segment of the very VIP! population we Serve/Support! Class A Members, provided for in our Articles of Incorporation (Summary) are People with a chronic Significant Physical Disability due to Development (PCSPDD), AND who have demonstrated, substantial "financial need" due to their inability to employ due to their disability (i.e. our "VIP! Members").

But because of Mission and capability to operate with high efficiency and effectiveness (Results), we can, within our IRS 501c3 "Tax-Exempt" status, ALSO serve VIP!s as "Clients". In such case, the VIP! has participating "3rd Party" "Stakeholders", or "Sponsors" (whether "State appropriated, or Private Payor (obligated) or OFFERED by a Private "Sponsor"). In any case, either such "Sponsors" or themselves DO NOT meet the LOW finanical Income and Network requirement. In such case we can Serve/Support almost identically, with the following provisos: 1)we cannot allocate any DONATED FUNDS OR GOODS (THINGS! program) to Serve/Support such "Clients"such, and 2) as long as such service and support is provided at reasonable "at-or-below" market-rate "Services for Fees", and 3) as long as such "Client Revenue" does not become a predominant source of our Total Revenue.

In either case, we require the following REQUIREMENTS of ALL VIP!s, whether qualified as "VIP! Members (Class A)" OR also (or exclusively) as "VIP! Clients" - pursuant to our Articles, Bylaws and these Policies.

* Please NOTE: Any of the following requirements MAY be WAIVED OR MODIFIED, as required for our compliance with a VIP!'s State-appointed Agency (i.e. "Government", and "statutory") funding source(s), as well as aH4L! compliance to certain "service and other Operating Procedures", AS is mutually agreed on by aHAND4 Life! ® via a separate written instrument ("contract") duly executed by the Officers of that Agency and aH4L! who are authorized to commit their Organizations funds and resources to the levels specified in such contract.




Section II. Requirements. *


ALL the following must be TRUE/Affirmed in "Registration", signed in "Application" (or Service Agreement).

VIP! MEMBERs & CLIENTs MUST DISCLOSE and AGREE AS FOLLOWS:


Section II A) Developmental Disability ("DD") Qualifications


VQ II) A) 1) HAVE the expressed written Consent of their Parent, Legal Guardian or Conservator if you are a MINOR (Age 16 to 17+), OR if an Adult AND have Legal Guardian or are Conserved, and provide their full Name (identity) and Contact Information. Children needing Education or preparation for Adulthood are a MAJOR FOCUS for the Organization!

VQ II) A) 2) ATTEST to and provide Govt document evidence of Tax-filing Status (e.g. IRS form 1040 Lines 1-5, e.g. Single, Married, etc)

VQ II) A) 3) ATTEST to your support for the Organization's Articles, Policies, Mission, Vision, Leadership, Staff, etc.

VQ II) A) 4) HAVE a written Medical Disability Diagnosis & Letter from a US licensed Medical Doctor (Physician); which state(s) your disability is due to "Development"as defined in our KEY DEFINITIONS; AND will supply copy of supporting documents confirming your Disability meets the following LEVEL I & II CRITERIA:

VQ II) A) 1) i). (LEVEL I) - is attributable to primarily (though not necessarily SOLELY) a Physical Impairment.
VQ II) A) 1) ii). (LEVEL I) was manifested before I turned 22 years old.
VQ II) A) 1) iii). (LEVEL I) is chronic, that is, medical professionals believe is likely to continue many years (>2)
VQ II) A) 1) iv). (LEVEL I) afflicts me SUBSTANTIALLY, AND in AT LEAST 3 ADL's (Activities of Daily Living) as follows:
VQ II) A) 1) iv) a). (Level II) my ability to care for myself
VQ II) A) 1) iv) b). (Level II) my ability to receive, or express language and communicate
VQ II) A) 1) iv) c). (Level II) my mobility, that is, my ability to be independently mobile
VQ II) A) 1) iv) d). (Level II) my ability to live fully independently, on my own, safely, in a basic life
VQ II) A) 1) iv) e). (Level II) my ability to provide for my own, basic economic self-sufficiency

  • HAVE a written standardized Psychological Assessement from a US Licensed Psychologist that states I HAVE "Normal" or "Near-Normal" Intellectual/Cognitive ability, AND "Normal" Emotional Behavioral ability ; and will supply copy of supporting documents confirming your Mental Cognitive and Behavioral ability is:

    1. with an IQ greater than (>) 60
    2. I am of "Normal" Emotional Behavior, which I confirm are ALL true:
    a. I am NOT affected by any Behavioral Disease, Disorder, and/or Psychosis
    b. I DO NOT have, NOR have I ever had a diagnosis as stated above, by a Physician or Psychologist
    c. I AM NOT avoiding Assessment or Treatment, or self-treating for any items listed above



  • Section II B). Personal Qualifications & Agreements!*


    VQ II) B) 1) BE of age to benefit from aH4L! Programs, which are all designed at maintaining an individual in their own home as long as possible. Therefore, an individual MUST BE at least of School Age and able to benefit initially from aH4L! Tutoring Programs, typcially Age 12 or older, though as early as six (6) years is possible as our Staff resources exist. Then typically age 17 to benefit ADDITIONALLY from all the Programs designed specifically toward entering a "Supported Living Environment" outside the Family home, being independent as possible legally, financially, finding/selecting a home and managing it, Caring for oneself and with additional Caregiver(s) beyond Parent/Family, and providing for their own "things". Our Service age ends when an individual must enter a care facility, typically later in life, though could be any age after becoming Member/Client.

    VQ II) B) 2) BE a Citizen of the United States of America (born in the US or US Territory), or formally "Naturalized" under US Law); AND

    VQ II) B) 3) HAVE reviewed and be supportive (written affirmation) of our Articles of Incorporation, Website Programs, Vision, Mission, Public Charity Status and Focus, General Focus, Leadership, Planned Events and Milestones, Organizational Structure and Operation Plan as published, or must submit any noted point(s) of non-support, and why, in Application (Contract Negotiation for Clients). Some limited dissent may be accepted but must be noted in advance; AND

    VQ II) B) 4) BE SUFFICIENTLY CAPABLE of managing one's own affairs Physically, Emotionally and Cognitively, with appropriately trained, professional Caregiver assistance (aH4L! or other equivalent) to ascribe to, and benefit from aH4L! Programs as provided and enrolled. Such individual MUST meet the SPECIFIC REQUIREMENTS listed in the Section below, which in summary are: have a DEVELOPMENTAL DISABILITY as assessed by a licensed, USA Specialist Physician, and fitting one of the Categories of our Service as stated, which are currently listed in "The Need is GREAT!" page, which is based largely on the US Health and Human Services (HHS) Administration on Community Living (ACL), AND US Social Security Administration (SSA) Health Definitions which we support. As well, MAY NOT, at any time, be found "incapable" by reason of a mental infirmity or mental disease or disorder or psychosis, or have IQ less than (< sixty-five (65) - all such as assessed by a licensed Professional Psychologist; OR MAY NOT BE subject to or in process of determining such, AND

    VQ II) B) 5) HAVE DEMONSTRATED interest in aH4L! improving the lives of OTHER SUCH people who are less advantaged as they (PSPD) in society, and as Member, willing to use their single vote contientiously and in good faith to elect the best leaders possible each election for the long-term interest of the Organizatioon and its continued Service as a US Public Charity, Articles, etc. as noted in the bullet above..

    VQ II) B) 6) AGREE to a basic CODE OF ETHICS (Ethical Conduct), which is: 1) to live as a "good neighbor" at all times while working with aH4L! in their home living situation, AND 2) be respectful of others and act, at all times, 3) becoming, as a positive influence in the Community, with this Organization, supporting it becoming a recognized and respected Leader in the Non-Profit Industry and in particular the "Best-in-class" Public Organization with respect to providing valued Services to people with a chronic, significant physical disability. This also includes, but is not limited to, 4) the individual MUST NOT HAVE a CRIMINAL record, that is, a government record MAY NOT indicate you have been arrested for, or convicted of (or pled guilty or no‐contest) in or of a criminal offense (felony), NOR are you in proceedings of such pending action, inside or outside the United States.

    VQ II) B) 7) NOT BE currently subject to any order, including restraining, judgment, or decree, which has NOT been subsequently reversed, suspended, or vacated by a court of competent jurisdiction, that either permanently or temporarily enjoins, bars, suspends, or otherwise limits your involvement with or against any person or business, including in the human services sector, or commercial goods, or with children, or involved with securities, financial, health, residential or the legal fields.

    VQ II) B) 8) NOT BE undischarged Bankrupt, or in bankruptcy proceedings; AND

    VQ II) B) 9) DISCLOSE whether you have a government record indicating you have judgment or lien against you or a property you own AND if so, must accept participation in aH4L!'s aHAND4 FINANCE! Program thru good faith attempt, or successful completion, with goal to fully and promptly settle such within available and developed means (typically 1-2 years or less).

    VQ II) B) 10) NOT HAVE been found formally, in writing, of significant misconduct, or discharge/expulsion, from or by any Organization , that is, by any employer or educational, real estate, or other related business professional or charitable organization, or Advisory Board thereof, to be in violation of any published Corporation, or Organizational Policy, to received any formal disciplinary hearing resulting in any corrective action(s), or resulting in discharge from status as Member, Client, Student, or Duties or Position if employed; or have had any practicing license(s) equivalently suspended or revoked.

    VQ II) B) 11) If you are an aHAND4 Life! ® Employee must also be an employee in "Good Standing", that is, must meet the Conduct clauses above, including may not have been on or currently in "probation".

    VQ II) B) 12) IF applying for or MEMBER, MUST proceed willingly and cooperate in the Nomination and Election Process pursuant to our stated Policies.

    VQ II) B) 13) IF applying for or MEMBER, and IF later, in due time, NOMINATED for Director, must meet and accept all qualifications and requirements for such Nomination and Election, and then IF Elected Director by Class A, must be capable of accepting and executing our Directors Service Agreement, as Directors are held to a higher standard yet than general Class A Membership. This includes that the Individual, upon accepting Nomination, must agree, if Elected, to be responsible for and pay timely, in advance, the PREMIMUM INCREASE in Annual Membership Dues, per Policy in effect at the time of Election. Typically this is 20-50% greater than standard Dues.

    VQ II) B) 14) APPLY Individually - no Group/Family Dues rates are offered. We need to establish individual compliance with our Requirements, and offer very low Annual Membership Dues. Thus, parent/child, sibling, married or otherwise related Member Candidates MUST Apply and maintain an individual Membership. Thank you in advance for your understanding.



    Section II. C) FINANCIAL Qualification


    VQ II) C) 1)aHAND4 ® Life!, by our experience with applicable Codes and Regulations and the needs of business, has created four (4) VIP! "Types" involving "Member" and "Client" Status. We work diligently with VIP! Candidates to qualify each, as possible, in ONE (1) of the following:

    a) VIP! Gold-1 ("Member" status, Unsponsored)
    b) VIP! Gold-2 ["Member" & "Client" status (Sponsored, Government or Private Payor)]
    c) VIP! Gold-3 ["Member" & "Client" status (Sponsored, Private Party)]
    d) VIP! Gold-4 ["Client" status (only, Non-Member)]



    Section II D) Program Interest(s)!*


    • PROVIDE indication of Programs you NEED and believe you, or many similar PSPD, could or would importantly benefit from. This is done initially in Registration (CHECK ALL), and is for our mutual preliminary understanding of interest and need in our Programs. As well, such MUST be renewed annually thereafter.

    • AT THE TIME OF BEGINNING MEMBERSHIP, participate in a two short kickoff meetings with aH4L! Staff, typically initially by Televideo Conference for one (1) to two (2) hours depending on Needs, followed by a short (one (1) hour meeting) Onsite at the Service Location (Parent or Individuals own home) to "finalize" "YOUR Life Plan!", and participate diligently, expressing your needs and cooperatively with aH4L! Staff, annually, thereafter.
