Re: VIP! Information (Member & Client, HIPPA Compliance)

aHAND4 Life! necessarily works with our VIP!'s "Protected Health Information (PHI)". We must be provided such to Qualify VIP!'s for Services, and then use such in initial and annual Assessments to determine which Services can be applied, and how, and thereafter throughout case management.

Therefore, our systems are required to be "HIPPA" compliant, and so our systems are designed to be. Firstly, all information is managed on a "need to know" basis. All Staff which may come in contact with such PHI, receive in advance, the necessary training in HIPPA Compliance. As well, we utilize a major, reputable web-hosting provider to securely store our website (https://, etc.) and collected Registrant, Applicant (including VIP!s) data Media Temple, owned by GoDaddy. We also assure provide Secure, offsite, backedup storage by a Service provider which provide such services for some of the leading Medical Hospitals and Medical Centers in the nation (we cannot mention who for security reasons). We have established a roadmap for HIPPA Certification as well.

A number of the provisions below relate to HIPPA compliance, and where applicable aHAND4 Life!implements such to such level of compliance. Where HIPPA compliance is NOT necessary, such as even for "Mission Critical" information, we do NOT implement HIPPA Compliance - though for such we STILL provide a HIGH DEGREE of security measures and precautions, to prevent things as "hacking", "fraud", "theft", and the like - measures we trust our Members and Clients and their families well appreciate in this modern "cyber" world, and their selection out of the many of a valued Service provider as aHAND4 Life!

Re: General Information Use

Website or Mailed In

aHAND4 Life! respects the privacy of our Interested Parties, Members and Clients. We will treat all the information you supply with great care and confidentiality and security. The information aHAND4 Life! requests via the web, is typically only general Personal Information REQUIRED for: 1) General Registration, OR 2) Introduction to the Membership process, so we can determine if we can effectively serve your interest. Such information is generally known by the Public, or as a close friend or Family member would know, but we request it anyway for your Affirmation.

aHAND4 ® Life!'s collects "Web User" or "Nominee" or "Applicant" information, such as submitted through aH4L! websites or mailed to our Corporate Headquarters, ONLY used for the purposes for which it was collected.

All information submitted electronically or mailed to aHAND4 ® Life!'s, ONCE IN OUR POSSESSION, and thru "Denial" or "Termination", are treated STRICTLY Confidentially, on an internal "need-to-know" basis only, and otherwise treated with our fullest security and confidence. We use secure servers, of modern vintage, so among the latest commercially available and commonly used by small non-profit Organizations's as ours. We also strive for HIPPA-like compliance of any Medical information submitted, which is minimal, to demonstrate compliance with our Policies. Such information IS NEVER released outside aHAND4 ® Life!'s without the expressed written consent of the owner of said inforation.

For any candidates we "Deny" Membership, or Members cancelling or having the Membership Terminated, at any stage along the "Regration", "Application" or Service or Membership process, we PERMANENTLY discard (shred, permanently erase from computers, etc., per HIPPA guidelines) all Registration and Application materials as a matter of policy - THEREFORE WE WILL NO LONGER HAVE ANY record of it, and any FUTURE interactions MUST BEGIN anew.

Financial, including Credit/Debit Card

Personal financial information submitted, and any credit card information (number, expiration date, CVC code, etc), are only collected as required per our Policies to effect the desired "financial transaction" - such as Donation, Membership Dues, etc, whether we process such manually, or via aH4L's! online website. This information is NEVER maintained by aH4L!, and subsequent to transaction processing, is immediately PERMANENTLY discared. All online transactions are securely transmitted using SSL encryption. If the user utilizes our Paypall sub-service, all Paypal Policies apply, and we believe such offered by Paypal are among the latest state of the art in online financial transaction processing. Therefore, IN NO CASE, does aHAND4 ® Life!'s accept any responsibility for any issues encountered by these "third-parties", as we have no control over it. We will, however, upon written request of the financial "donee or member", comply with all reasonable requests for information leading to resolution and answer of questions related to such financial transactions.

Advertising Policy

aH4L! does not host any form of advertising from our websites or email communications.

Cookies

A cookie is a piece of data stored on the user's computer tied to information about that user. aH4L! uses both session ID cookies and persistent cookies. For the session ID cookie, once users close the browser, the cookie simply terminates. A persistent cookie is a small text file stored on the user's hard drive for an extended period of time. Persistent cookies can be removed by following Internet browser help file directions.

The primary function of cookies for aH4L! is to facilitate use of our websites by our users, and to enrich your online experience with us.

Persistent cookies are used to store the user's registration identification information from their basic registration, to facilitate their logging in to their personalized view of aH4L! websites. Persistent cookies enable us to track and target the interests of our users to enhance their experience on our sites. See the Profile section.

Session cookies remember what chapter you are registered into or visiting online and who you are if you go from one aH4L! website to another in a session. This way we are able to cut down on the number of logins required when you visit various aH4L! websites during any given session.

If users reject cookies, they may still use our sites, and all the informational resources provided through them. Their registration-based personalization options and the capacity of their browser to keep track of what chapter they are visiting online as they go from one of our sites to another, is lost if the user rejects cookies. aH4L! uses a tracking utility provided by a 3rd party vendor to track usage of specific web pages and other resources provided through our website. The vendor sets cookies as a method for compiling site usage information but these cookies are only used to compile demographic models of site usage and are not tied to the user's personal information.

Membership Name/City Disclosure

* Please NOTE, we hold the Name and Identity of all Members in obscurity - unless a Member becomes elected Director, OR the Member signifies in writing their giving level qualifies and they wish to include their Name and/or State in the Champion Recognition listings of Platinum, Gold, Silver or Bronze Contributors. The intent of this Policy is to provide for confidentiality and/or anonimity where possible, or recognition where required (Director), or where desired (otherwise). Recognition helps the Organization grow and attract Membership (all Categories), Leadership and Staff.

Financial and Medical Records Requests

aHAND4 ® Life!'s NEVER sends UNSOLICITED Requests for Registration OR Membership Information, without you ALREADY being involved intimately in the process, and without including clearly identifiable information that you are already aware of regarding said process. For online Membership Applications in particular, where some Confidential Information IS requested, we provided secure servers, password control, authentication, and email exchange with you, over a very limited period (typically several hours), for information exchange. We also utilize high bit level (128bit minimum) encryption AND industry norm secure communications technology. You also are required to provide a temporary MEMBERSHIP Application Code confirmed by a Fiduciary Part of our Organization, typically the President, as sent from a secure email address at aHAND4 Life! .org,

aHAND4 ® Life! NEVER requests SPECIFIC Confidential Information as FINANCIAL OR MEDICAL Records via the Web. This information includes bank account or credit/debit card numbers, social security numbers, employer numbers, specific annual income or Tax Records, your specific medical diagnosis, doctors information or any other of your Patient Records which are Private and protected under the US HIPPA (Health Information and Protection Act). We also DO NOT request Names of Family, Caregivers, Pets, prior Schools or other various highly confidential information which could be used in passwords, etc. NOR do we ever request to ARRANGE DELIVERIES that you are unaware of ahead of time. All of these we arrange with you in direct, secure communication, in writing and with personal phone contact, and typically involving US Mail for completion of formal written contract agreements. We utilize multiple step processess, and controls. If you EVER receive a request for such information, and are unsure of the Origin, PLEASE DO NOT reply! First CONTACT aHAND4 Life! in a trusted manner, or have another trusted Party to assist you in your determination. Please see ADDITIONAL related Policies below (Privacy, Security, Disclaimers, etc.). Registration, Membership & Clientship are performed at the provider's (you) sole discretion.

Newsletter

If a user wishes to opt-in subscribe to our e-newsletter, we ask for contact information required for basic registration to the site, consisting of a working email address, a zip code and a password.

As indicated above, Communications from our site our limited to specific purposes related to your ALREADY initiated Registration or Membership, including easily identifiable information YOU provided immedaitely previously, in that process. This includes any Personal Confidential, Financial, or Medical record information. In each case, we attempt to limit the information to that essential to the process, and defer the remainder to written hardcopy, provided to you via US mail.

Sharing

Legal Disclaimer

Though aH4L! makes every effort to preserve user privacy, we may need to disclose personal information when required by law wherein we have a good-faith belief that such action is necessary to comply with a current judicial proceeding, a court order or legal process served on our website. aH4L! may need to share personal information with third parties in other limited circumstances. For example, we may release personal information to third parties to perform services on our behalf such as: advising you of our upcoming events, products, partnership offers, volunteer opportunities, and health-related information or processing credit card payments. aH4L! may also make your name and postal address available to other nonprofit organizations whose services or activities may be of interest to you. However, third parties retained by aH4L! to provide such services are obligated to maintain the confidentiality of this information and are not authorized to use this information for any purpose other than providing those services. If you would like to opt-out of having aH4L! share your name and postal address with third party organizations, please email admin@lls.org and type 'do not share' in the subject line of your email.

Third Party Intermediaries

aH4L! uses various 3rd party transaction services to process online donations. aH4L! provides the option of receiving certain of its online booklets, brochures, and fact sheets by mail through request forms on our websites. These materials are shipped by the US Postal Service.

Additional services for Light The Night Incentives, ordering from the aH4L! e-store also utilize 3rd party vendors for ordering, fulfillment and customer service.

We will only provide these companies the information they need to deliver the service and prohibit them from using that information for any other purpose. These companies/organizations do not share or use personally identifiable information that may be obtained from these transactions for any secondary purposes.

Open Forums

Message boards and chat rooms may be available to visitors of the Site. The forums will identify visitors as requested by the visitor. Please remember that any information you disclose in these areas becomes public information. Accordingly, you should always exercise caution when deciding to disclose personal information.

Notification of Changes

If aH4L! changes its privacy policy, changes to this privacy statement will be posted, with prominent links to the new policy positioned on the aH4L! homepage (https://www.ahand4life.org), and other places we deem appropriate, so our users are always aware of what information we collect, how we use it, and under what circumstances, if any, we disclose it. aH4L! will use information in accordance with the privacy policy under which the information was collected.

Our Commitment to Children's Privacy

Protecting the privacy of the very young is especially important. For that reason, we never collect or maintain information on our website from those we actually know are under the age of 13.

We are dedicated to safeguarding any personal information collected online and to helping parents and children have positive experiences on the Internet. aHAND4 Life! (aH4L!) goes to great lengths to protect the confidentiality and integrity of all the personal information share with the Organization via the web.

aH4L! treats highly critical data such as credit card and Social Security numbers, as well as your SIGNIN Password as critical assets that must be guarded at all times against loss and unauthorized access, online and off-line. Thus, we utilized advanced, industry-standard computer security encryption (hashing) techniques, which means no human being within aH4L! ever sees such information at any time. As well, our strict internal administrative Policy highly limits the number of employees who have access to even the encrypted data.
